Abstract

Hospitals are always required to improve the existing services in order to be competitive. Service improvement can be done by giving the best to the patients in every aspect, especially services in terms of patient care. However, currently patients at the Universitas Sumatera Utara (USU) Hospital often complain that they have to wait for a long time before a doctor examination in the clinic it to take place. This is because it takes a long time for the medical record to reach the clinic. Therefore, it is necessary to risk analysis that occurs in the process of medical record section to identify the cause most potentially causes the process in the medical record to be long. The study was conducted by applying FMEA and fishbone diagrams in analyzing the risks associated with the failures impact in the process flow of medical record documents. The result shows that the highest RPN value of 288 where the main cause of failure was due to files not brought back into place and the arrangement of files was not in order. Both of these result in a long process of file search that has an impact on the patient's treatment process and they become top priorities. © 2019 IEEE.
